AUSTIN – The Texas Department of Licensing and Regulation entered Agreed Orders against twenty-three individuals.
Terry Lynn Mucker, d/b/a Hair Doctor Barber Shop in Houston: $3,000 Administrative Penalty
-Hair Doctor Barber Shop operated as a barber shop without a permit
-Mucker leased space to five (5) individuals without booth rental permits
-Mucker employed two (2) unlicensed individual
Tony John Lucca & Lorraine Simmons, d/b/a Tony’s Hair Company in The Colony: $1,500 Administrative Penalty
-Lucca and Simmons failed to renew Barber Shop Permit in
a timely manner
Alfredo Sierra, Jr., d/b/a Dallas Finest in Dallas: $1,500 Administrative Penalty
-Dallas Finest operated as a barber shop without a permit
-Sierra employed an unlicensed individual
William Hardaway, d/b/a Bilmor’s of Fort Worth: $1,500 Administrative Penalty
-Hardaway leased space to an individual without a booth rental
Permit and the individual was practicing with an expired
Class A Barber license
Keith Thomas, d/b/a The Third Barber Shop in Houston: $1,000 Administrative Penalty
-Thomas leased space to two (2) individuals without booth rental permits
Helen Blake, d/b/a The Haircut Factory in Tyler: $750 Administrative Penalty
-The Haircut Factory operated as a barber shop without a permit
Renee Keeton, d/b/a The Barber Shop in Corsicana: $500 Administrative Penalty
-Keeton allowed an individual to perform barbering services with an expired license
Daniel Cervantez, d/b/a Hair Blast, Etc. in Mercedes: $500 Administrative Penalty
-Hair Blast operated as a barber shop without a permit
Noralisa Nino, Hair Blast of Mercedes: $500 Administrative Penalty
-Nino performed barbering services in barber shop that did not hold a Barber Shop Permit
Willis Dewitt, of Missouri City: $500 Administrative Penalty
-Dewitt performed or offered to perform services with an expired Class A Barber license
Leatha D. Patterson of Fort Worth: $500 Administrative Penalty
-Patterson performed barbering services in barber shop that did not hold a Barber Shop Permit
Maximino Ballez Cavazos of Schertz: $250 Administrative Penalty
-Cavazos rented space in a barber shop but did not hold a booth rental permit
Anthony Jerome Taylor of Fort Worth: $250 Administrative Penalty
-Taylor rented space in a barber shop but did not hold a booth rental permit
Martin Garcia, d/b/a Old School Barber Shop: $1,000 Administrative Penalty
-Garcia had one (1) container of “Steri-dry,” a formaldehyde fumigant, on the premises of Old School Barber Shop
Stacy Roznovak, d/b/a Nails by Stacy in Taylor: $1,500 Administrative Penalty
-Roznovak failed to provide a department-approved sterilizer
-Failed to replace soiled disinfectant with freshly prepared solution
-Failed to clean equipment, implement, tools or materials before disinfecting
Martin Clark, d/b/a Clark’s Barber Shop in Marshall: $1,000 Administrative Penalty
-Clark failed to replace soiled disinfectant with freshly prepared solution
Esteban Flores, d/b/a Steve’s Barber Shop in Harlingen: $750 Administrative Penalty
-Failed to store implements in a clean, debris-free, dry environment
-Failed to properly clean equipment after each client
Marjorie B. Martinez, d/b/a Bemada’s Hair Design in Sealy: $750 Administrative Penalty
-Failed to remove and deposit soiled towels in a suitable receptacle
-Failed to clean equipment, implement, tools or materials after each client
Rodney Walker, d/b/a True Faiths Cuts of Stafford: $750 Administrative Penalty
-Walker failed to use an EPA-registered bactericidal, fungicidal and/or virucidal disinfectant at two (2) work stations in the salon
-Failed to store implements in a clean, debris-free, dry environment
Julian J. Gaucin, d/b/a Golden Gate Barber Shop in Brownsville: $500 Administrative Penalty
-Gaucin failed to use an EPA-registered bactericidal, fungicidal and/or virucidal disinfectant
-Failed to replace soiled disinfectant with freshly prepared solution
Eusebio Lumbrera, Jr., d/b/a Chevo’s Barber Shop in Dimmitt: $375 Administrative Penalty
-Failed to replace soiled disinfectant with freshly prepared solution
Cleveland Blair of La Porte: $250 Administrative Penalty
-Failed to store implements in a clean, debris-free, dry environment
Cynthia Marshall, d/b/a Red’s in Amarillo: $250 Administrative Penalty
-Failed to replace soiled disinfectant with freshly prepared solution
