Fifteen Agreed Orders Entered for Unlicensed Activity

AUSTIN – The Texas Department of Licensing and Regulation (TDLR) entered into Agreed Orders against fifteen individuals for unlicensed activity and sanitation violations.

Open for business with no Barber Shop Permit

Russell Louis Forrest, d/b/a The Barber Zone in Hempstead: $1,500 Administrative Penalty
-Forrest was present and waiting for customers but did not hold the a Barber Shop Permit

Bill M. Simpson, d/b/a Travis Hair Styling II of Austin: $1,000 Administrative Penalty
-Simpson was open for business but did not hold the a Barber Shop Permit

Hien Huu Nhgyen of Fort Worth: $750 Administrative Penalty
-Nhgyen worked in a salon that did not have a Barber Shop Permit

Barbra J. Chalmers, d/b/a Queen B’s Barber & Beauty Shop in Dallas: $750 Administrative Penalty
- Chalmers operated a barber shop without a Barber Shop Permit

Carl L. Williams Jr., d/b/a Barbershop in Jefferson: $500 Administrative Penalty
-Williams was observed performing barbering services in a facility that did not have a Barber Shop Permit

No Barber Shop Permit and an expired Class A Barber license

David Payne, d/b/a Payne’s Barber Shop in Sour Lake: $500 Administrative Penalty
-Payne performed barbering services with an expired license in a barber shop that did not hold a Barber Shop permit

Mitchell Newton of Tyler: $500 Administrative Penalty
-Performing services without a booth rental permit

Carlos Ray of Red Oak: $500 Administrative Penalty
-Performed services without a booth rental permit

Glen Earl Mosley of Fort Worth: $2,250 Administrative Penalty
-Mosley was available to perform barber services but did not hold the required barber license
-Rented space in a barber shop but did not hold a booth rental permit

Pedro Garcia, d/b/a Flew Barber Shop in Edinburg: $1,500 Administrative Penalty
-Garcia employed an unlicensed individual
-Failed to replace soiled disinfectant with freshly prepared solution

Seada Arefa of Chicago, Illinois: $1,000 Administrative Penalty
-Performed services without a license

Donnie R. Moore, d/b/a Freestyle: $750 Administrative Penalty
-Employed unlicensed individual

Gregory Dion Johnson, d/b/a Greggs Unisex in Red Oak: $750 Administrative Penalty
-Employed unlicensed individual

Sanitation Violations

Barbara Troha of Joshua: $2,000 Administrative Penalty
-Failed to use an EPA-registered bactericidal, fungicidal, and/or virucidal disinfectant in accordance with TDLR regulations
-Failed to replace soiled disinfectant with freshly prepared solution
-Failed to properly clean and disinfect equipment, tools and/or implements

Jose R. Castillo of Pasadena: $250 Administrative Penalty
-Failed to store implements in a clean, debris-free, dry environment
